You will be issued an Order ID. Please feel free to label your bag of consignment or donation items with the Order ID and your surname.

Please list all items and quantities for resale. You may choose to sell items on consignment or donate them to the P&C. A percentage of fees from items sold will go towards P&C fundraising initiatives. Please see the Terms and Conditions at the bottom of this form.
